Ohio Gas Association
Award of Merit

Purpose
To emphasize the Ohio gas industry's dedication to safety and to recognize employees who actively demonstrate a commitment to safety.

Qualifications
Ohio gas utilities who are active members of the Ohio Gas Association may submit nominations according to the following guidelines:

  1. There must be evidence that a nominee demonstrated initiative or skill in saving or attempting to save the life of another person.
  2. Nominations must be submitted for consideration involving actions taken to preserve a life during emergency or disastrous situations.
  3. The award is available to Ohio gas utility employees who perform life saving actions while engaged in the performance of gas industry activities or personal activities.

Nominating Procedure
Supervision should be alert to and report promptly to the company safety officer or designate any employee act that maybe considered an act of merit. Other employees knowing of a case for consideration in any Ohio Gas Association member utility should report details to supervision. The safety officer or designate within the utility shall investigate details of the case and complete the formal application.

Applications
An Ohio Gas Association Award of Merit application form shall be used for nominations. An individual application shall be prepared for each nominee. This report is provided in Adobe PDF format which means to you must have Adobe Acrobat Reader installed on your computer.) Documentation can be used for multiple nominees involved in a single case. Include all details of facts that will help establish a clear understanding of the case for making an aware. The checklist on the application form can be used as a guide for preparing documentation.

Appraisal and Approval
The nominating company will submit to the President of the Ohio Gas Association an application form with documentations of the case. The President will forward the application and case documentation to the Accident Prevention & Safety Training Committee (AP&STC) members for review and consideration. The AP&STC members will vote on the merit of the application at its regularly scheduled meeting. A majority affirmative vote is required for approval.

The chairman of the AP&STC will present the committee's recommendation to the President. The President will present approved applications to the Board of Trustees for final approval.

The President will notify nominating companies of the final disposition of all applications.

AWARDS

The type and design of the ward will be determined by the Accident Prevention & Safety Training Committee and the President of the Ohio Gas Association (OGA).

Recognition
Recognition of all Ohio Gas Association member companies whose employees receive awards will be made at the Annual Meeting. Posters will be prepared and placed in a conspicuous location. The posters will include a photo of the recipient, a description of the safety act performed, and the name of the recipient's company.

Presentation of Award
Due to the diversity with which member companies conduct their safety programs, optional methods for ward presentations will be considered.

  1. The Award will be presented to the recipient at the OGA Annual Meeting upon request by the member company. The request for presentation will be made on the application form prepared by the nominating company.
  2. Unless requested to present the award at the Annual Meeting, all awards will be sent to the recipient's company.
